A visit to the home
A service coordinator will visit the family home to meet with the participant and, with their consent, family members to discuss the participant’s needs, goals, and available support. The visit will help identify any concerns in the home and coordinate services that support the participant’s safety and independence. We will also review the service plan and address any changes in the participant’s needs.

How we work with a family
Finding the right support for your family can feel overwhelming. Our service coordination helps you understand your options, connect with resources, and plan your next steps. We work alongside you, keeping your family’s needs and goals at the heart of the process.
