We provide compassionate service coordination for people in the NHTD and TBI programs and Holocaust survivors, helping individuals and families access the support they need to live safely and independently at home.
NHTD
Nursing Home Transition and Diversion.
We help increase the hours, process all necessary documentation, and provide daily support for your loved one at home.
No fee to the family.
TBI
Traumatic Brain Injury waiver.
We help with documentation and with increasing hours.
It usually takes between 3 and 6 months.
No fee to the family.
Holocaust survivors
ComfortZone offers support for Holocaust survivors.
No fee to the family. Call us and we will explain the process.
How a family starts
- The family member calls us.
- We explain the whole process, and all the necessary documents and paperwork.
- Have information about your loved one ready, such as their medical condition.
- If you do not know which program you need, just call us. We will take it from there.
