Families often ask what services are available, who we help, how to get started, how long the process takes, and how coordinators can help them access support.

Questions families ask

What services are available?

We help families get more hours of care, help with paperwork and applications, and provide daily support in the person’s home. We help someone stay at home instead of a nursing home. We offer support for Holocaust survivors and support after a brain injury. The programs are NHTD (Nursing Home Transition and Diversion), the TBI waiver, and support for Holocaust survivors.

Who do you help?

Older adults and sick elderly people, especially people living with dementia, Alzheimer’s, and traumatic brain injury. Sons, daughters, relatives, friends, or neighbors can call on their behalf. If you are not sure which program fits, call us and we will take it from there.

How do I get started?

Call 718-808-1050 or 718-602-4028, or send a message. The family member calls us. We explain the whole process, and all the necessary documents and paperwork. Have information about your loved one ready, such as their medical condition. We respond within the same day.

How long does the process take?

For the TBI waiver, it usually takes between 3 and 6 months. Call us about timing for NHTD or support for Holocaust survivors.

How can a coordinator help?

A service coordinator will visit the family home to meet with the participant and, with their consent, family members to discuss the participant’s needs, goals, and available support. The visit will help identify any concerns in the home and coordinate services that support the participant’s safety and independence. We will also review the service plan and address any changes in the participant’s needs. We help increase care hours, process the documentation, and provide daily support.

Is there a fee?

No fee to the family.

Where do you provide service?

We provide service in the state of New York, including Brooklyn, Queens, Manhattan, the Bronx, Staten Island, Nassau County, Suffolk County, and Westchester County.

Which languages can we speak?

English, Russian, Spanish, Yiddish, Hebrew, Polish, Ukrainian, Creole, French, Uzbek, and Chinese.

What if I call and no one answers?

Please leave a message and we will call you back as soon as possible. We are here 7 days a week, and we respond within the same day.
